Failure analysis of the fractured wires in sternal perichronal loops.

چکیده
We report failure analysis of sternal wires in two cases in which a perichronal fixation technique was used to close the sternotomy. Various characteristics of the retrieved wires were compared to those of unused wires of the same grade and same manufacturer and with surgical wire specifications. In both cases, wire fracture was un-branched and transgranular and proceeded by a high cycle fatigue process, apparently in the absence of corrosion. However, stress anlysis indicates that the effective stress produced during strong coughing is lower than the yield strength. Our findings suggest that in order to reduce the risk for sternal dehiscence, the diameter of the wire used should be increased.

Potential risk of sternal wires.
OBJECTIVE To understand the potential fracture mechanism of sternal wires, we collected extracted stainless steel sternal wires from patients with sternal dehiscence following open-heart operations. Surface alterations and fractured ends of sternal wires were inspected and analyzed. METHODS Eight fractured and 12 non-fractured wires extracted from five patients (closure method: figure-of-eigh...
متن کاملFatigue failure analysis of stay cables with initial defects: Ewijk bridge case study
A failure assessment method of stay cables containing fractured wires is presented through an example of an existing bridge. The wires constituting the stay cables of the bridge contained many initial defects that were formed during the rolling process of these wires. Despite moderate loading, fatigue cracks started to grow from these defects, increasing their size, and finally resulting in fra...
متن کاملA biomechanical comparison of three sternotomy closure techniques.
BACKGROUND A biomechanical study of three sternotomy closure techniques (figure-of-eight stainless-steel wires, Pectofix Dynamic Sternal Fixation [DSF] stainless-steel plates, and figure-of-eight stainless-steel cables) was conducted to compare strength and stiffness variables in three clinically relevant loading modes (anterior-posterior shear, longitudinal shear, and lateral distraction). M...
متن کاملSudden pacemaker failure.
Two cases of sudden pacemaker failure due to fractured wires supplying power to the generator are described. Both devices were Kappa KD703 (Medtronic Inc., Minneapolis, MN, USA) implanted in a prepectoral subcutaneous pocket. In 2002, Medtronic issued an advisory regarding Kappa 700/600 devices. A number of these devices, all subpectoral implants, had presented with intermittent or no output du...
متن کاملSternal plating for primary and secondary sternal closure; can it improve sternal stability?
BACKGROUND Sternal instability with mediastinitis is a very serious complication after median sternotomy. Biomechanical studies have suggested superiority of rigid plate fixation over wire cerclage for sternal fixation. This study tests the hypothesis that sternal closure stability can be improved by adding plate fixation in a human cadaver model. METHODS Midline sternotomy was performed in 1...
